To schedule an individual or  couples appointment or to obtain additional information about any of these counseling services, please fill out the form below or give me a call. Appointment times are Monday through Friday from late morning to early evening. 

Once you make an appointment you will need to download the forms from the Helpful Forms page, fill them out and bring them to your first appointment. You will have a total of 4-5 forms to fill out.
